Dr. Toby Smith

Toby Smith, DO has been involved in the Post-Acute and Long-Term care industry for over 20 years. Initially involved with nursing home research at UCLA’s Borun Center for Gerontological Research, he decided to pursue a career in medicine to improve the quality of care for nursing home residents. He obtained his medical degree(D.O.) from Western University and did his residency training at the University of Hawaii in internal medicine as well as fellowship training in geriatric medicine. After training and working as a hospitalist, he was eventually hired as one of the first full time onsite nursing home physicians in Hawaii and worked for one of the largest nursing home companies in the U.S. He became a certified medical director(C.M.D.) through the American Medical Directors Association and has served as a nursing home medical director in multiple facilities throughout Hawaii. He is now a core faculty member for AMDA and also teaches as an assistant clinical professor of geriatrics at the University of Hawaii.Dr. Smith is ABIM board certified in Geriatric and Hospice/Palliative Medicine and is currently the Chief Medical Officer and Vice President for a nursing home provider group. He additionally provides his expertise as a medical director for hospice and the largest insurance company in Hawaii for palliative care.
